2006-2007 Forms

ALL participants in tryouts must fill out and turn in the forms listed below.

Form #1 - 2006-07 NERVA Membership Form - Click to download

All participants must fill out the NERVA membership form to be able to participate in tryouts.  

The form is an Adobe PDF document which you will need to print out (please do so on one sheet of paper, front and back).  Click here to Download a FREE copy of the Acrobat Reader from Adobe. 

Instructions for filling out the NERVA form:

Front Page:  You need to fill out SECTION I (name, address, etc.), plus the Grade and High School section of the JUNIOR OLYMPIC MEMBER section.  Enter "RI  JUNIORS VOLLEYBALL" in the JO Club section.  DO NOT fill out the Team Name.  Select your Gender, Division, and Type of Membership from the options in the check box section.

Back Page:  Please read all of the information provided.  Players who are 18 or older must sign the Participant's Signature line, and date it.  Those under 18 need to have a parent/guardian sign the bottom of the form.

 

 

Form #2 - 2006-07 MEDICAL HISTORY & RELEASE FORM - Click to download

This document is a Word file.  There are two pages.  Please print on one page front and back if possible.  If you have trouble printing the document we can try to send it to you in an email.  Instructions for filling out the Medical History & Release Form:

Front Page:  Please fill out completely and legibly.  We must have the participant's insurance information filled out.  Be sure to sign both sections. 

Back Page:  Please fill out completely and legibly.  If child is up to date on all immunizations please write "current". 

Please also designate if it is okay for your daughter to receive Motrin, Pamprin/Midol, Pepto-Bismol or Rolaids on the form where indicated.
